Instructions
We hope that you will take a few minutes to participate in this survey which has been approved by LTC Nancy Ruffin (Deputy Director Family Programs, DA Family Advocacy Program Manager). The purpose of this survey is to:
- determine what training materials and related resources have been most useful to you in providing your installation Family Advocacy Program (FAP) prevention services,
- identify how these existing FAP prevention resources are being used,
- consider your suggestions on what other evidence-based information would assist you with your work, and
- obtain additional feedback regarding your work with Soldiers, DA Civilians and Family members.

All of the information that you provide is confidential. This survey will be housed on a secure server maintained by the Army and participants' responses will not be tracked back to them. Only the Cornell staff involved in preparing the information for analysis have access to the completed survey questionnaires. For the final summary report, only group statistics are provided and names or other demographic identifiers disclosed in any open-ended comments are removed. The results from this survey will be provided to HQ FAP staff.

When completing the survey, please answer these questions based upon the management of the overall FAP program which incorporates the work of other FAP staff, contractors and military and community partnerships at your installation.
